Important Counseling Forms
All forms below can be picked up from the Counselor's Office.
Every student must have a LOSFA Consent form returned before senior year. This is important to have uploaded to JCampus for the student to be eligible to receive the TOPS or TOPS Tech award if they qualify. The form must be signed by a parent/guardian.
Every student starts on the University pathway as a freshman. At the end of sophomore year, they decide if they want to stay on the University pathway or switch to the Jumpstart Pathway. This form must be signed by a parent/guardian for the student to switch to Jumpstart.
If a student wants to graduate high school in 3 years as opposed to 4 years, they need to meet with their counselor as soon as possible so they can plan out the courses they'll need to take. They also need to pass their Leap 2025 requirement before the end of their sophomore year. This form must be signed by a parent/guardian for the student to attempt early graduation.
